Özet

BACKGROUND: Our study aimed to evaluate the incidence of postoperative CD recurrence in patients with UC who underwent colectomy. METHODS: Patients who underwent surgery with the diagnosis of ulcerative colitis at Cukurova University Faculty of Medicine between 2010 and 2017 were analyzed retrospectively. Thirty patients were included in the study. RESULTS: A total of 30 patients were included in the study, 17 men (57%) and 13 women (43%). The patients were followed for an average of 10.2 years with a diagnosis of UC. The patients were followed for an average of 6.3 years after the surgery. The majority (80%) of the patients had extensive-type and 20% had left-type ulcerative colitis. Ileal pouch-anal anastomosis was performed in 67% of the patients, and end ileostomy was performed in 33% of the patients. Around three-quarters (73%) of the patients were experienced with biologic agents, and 27% had never taken biologic agents before. CONCLUSIONS: In conclusion, it remains unclear if de-novo CD develops in patients who undergo total colectomy for UC, if silent CD is present in the background from the start, or if there is misdiagnosis from the outset. The fact that fistula development occurs after the 5th year reveals the importance of long-term follow-up for this patient group. It is vital to anticipate patients who will develop CD after colectomy. Thus, it is important to know the group to whom aggressive anti-inflammatory treatment will be given before colectomy.
