Objective: This study aimed to evaluate the demographic, toxicological, and forensic characteristics of cases in which illicit/stimulant drugs were detected in forensic autopsies performed at the Adana Group Presidency of the Council of Forensic Medicine between 2021 and 2025, as well as deaths due to intoxication with these substances. Materials and Methods: The study population of this retrospective descriptive study comprised 4,163 forensic autopsy cases performed between January 1, 2021 and December 31, 2025. Of these, 637 cases in which illicit/stimulant drugs were detected by postmortem toxicological analysis constituted the main study group and were retrospectively evaluated. Cases were assessed in terms of age, sex, cause and manner of death, location of the incident, substances detected, and multiple-drug use. Deaths due to illicit/stimulant drug intoxication were analyzed separately. Pregabalin and gabapentin, which have therapeutic indications but also potential for misuse, were evaluated separately from classical illicit/stimulant drugs. Results: Illicit/stimulant drugs were detected in 15.3% (n=637) of all autopsy cases. Of these cases, 92.0% were male, and the mean age was 33.5 years. The most frequently detected substances were amphetamine/methamphetamine (68.7%), synthetic cannabinoids (29.5%), and heroin/morphine (17.1%). Multiple illicit/stimulant drugs were detected in 36.4% of the cases and in 64.4% of those who died from intoxication. During the study period, the cause of death was illicit/stimulant drug intoxication in 177 cases (27.8%); the proportion of intoxication-related deaths among all forensic autopsies increased from 3.3% in 2021 to 7.2% in 2025. Although amphetamine/ methamphetamine was the most frequently detected drug group over the entire five-year period, an increase in the detection frequency of synthetic cannabinoids was observed in 2024 and 2025. Pregabalin and/or gabapentin was detected in 41.2% of intoxication cases. Conclusion: The marked predominance of young adults and males, the frequent detection of multiple drugs, and the recent increase in intoxication-related deaths are noteworthy among forensic autopsy cases positive for illicit/stimulant drugs. The changing detection pattern of synthetic cannabinoids and the co-detection of pregabalin/ gabapentin in intoxication cases highlight the importance of up-to-date postmortem toxicological surveillance encompassing not only classical illicit/stimulant drugs but also prescription medications with misuse potential.
