By 2050, the proportion of individuals aged 65 and above is expected to increase significantly, necessitating the development of strategies to enhance the health and well-being of older adults. This chapter explores evidence-based approaches to improve future health outcomes for the elderly, focusing on preventive healthcare, chronic disease management, mental health support, social engagement programs, and policy interventions. A comprehensive review of global public health strategies, case studies, and policy frameworks will be conducted to identify effective practices for addressing the needs of aging populations. Key strategies to be proposed include preventive healthcare measures such as routine health screenings, vaccination programs, and guidelines for nutrition and physical activity. Chronic disease management will emphasize personalized care plans, integrated healthcare models, and coordinated services. Mental health support will focus on addressing loneliness, depression, and cognitive decline through community-based programs and therapeutic interventions. Social engagement programs will aim to promote active aging and reduce social isolation through technological solutions and community activities.
